A Digital Intervention Platform Created for Postpartum Moms is Improving Outcomes and Saving Lives

Atlanta, Georgia: Liberty Regional Medical Center, in partnership with GoMo Health, celebrated the success of the Mom’s Heart Matters platform at a community event on August 14, 2024. Mom’s Heart Matters is an interactive digital intervention that leverages behavioral and cognitive science to improve perinatal wellness and reduce the rate of hypertension-related mortality. Since the launch of this patient-influenced intervention, Mom’s Heart Matters has tracked numerous evidence-based cases of reduced adverse events and life-saving interventions.

The platform includes remote patient monitoring (RPM) that utilizes Bluetooth enabled blood pressure cuffs to collect real-time patient data and deliver it to the care team to enable immediate response as/if needed. This combines with delivery of resources that are personalized to each participant to address clinical needs and social drivers of health for better overall care.

In the initial pilot, there was a 95% retention rate among all enrolled participants. Of the participants opted into remote blood pressure monitoring, 23% had escalations of which 100% were addressed with real-time interventions. In addition to blood pressure monitoring, the platform screens for depression (PHQ-2 and PHQ-9), maternal functioning (Barkin Index of Maternal Functioning), and substance use (SBIRT) to identify each mom’s needs and connect to necessary resources.
